Tranquility Hospice seeks a Medical Social Worker in Delaware, OH, and surrounding counties. This full-time position involves Monday–Friday 8:30 a.m. – 5 p.m.

Qualifications

• Must be licensed by the State of Ohio.
• MSW degree from a CSWE-accredited school.
• BSW with 1 year experience and supervision by an MSW.

Responsibilities

• Conduct psychosocial assessment within 5 days of admission to the hospice program.
• Participate in IDG meetings and care plan development.
• Maintain and submit initial evaluation, care plan and daily notes.
• Evaluate patient/family response to social work interventions.
• Coordinate weekly visits with Case Managers to facilitate coordination of care.
• Participate in IDG and QAPI meetings and activities.
• Assist in discharge planning as needed.
• Act as consultant to the health team on social, emotional, and environmental factors.
