Job summary

Tranquility Hospice is seeking a Medical Social Worker to provide psychosocial support and social services to hospice patients and families under an individualized Plan of Care. You will collaborate with the hospice interdisciplinary group to assess needs, connect families with resources, and promote care continuity throughout the end-of-life journey.

Qualifications

• Must hold Ohio license to practice social work (LSW/LISW) as applicable to education and scope of practice.
• Must meet applicable federal hospice personnel qualification requirements for social workers.

Responsibilities

• Complete psychosocial assessments and contribute to Plan of Care within required timeframes.
• Assess emotional, social, financial, environmental, cultural, and psychosocial needs and resources.
• Identify psychosocial concerns, goals and support needs related to illness and hospice care.
• Coordinate with IDG and participate in development, review, and updates of the Plan of Care.
• Provide counseling, advocacy, and resources to patients and families; assist with discharge and transfer planning.
