Job summary

Heart to Heart Hospice in Florida is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of a registered nurse. You will support transfers, ambulation, and daily activities, while maintaining infection control and reporting changes.

This full-time role requires a current CNA license, a high school diploma or GED, and at least one year of related experience in hospice, hospital, or long-term care.

Qualifications

• CNA license is required and must be current.
• High School Diploma or GED preferred as educational baseline.
• Experience in hospice, hospital, long-term care, or home health is preferred.

Responsibilities

• Provide direct patient care under RN supervision.
• Assist with safe transfers and ambulation per policy.
• Report and document changes in patient condition for continuity of care.
• Practice infection control principles at all times.
• Communicate respectfully with patients, families, and staff.
• Submit all documentation in a timely manner.
• Perform other duties as required by the facility.
• Ensure compliance with laws, regulations and policies.
