Gentiva seeks a compassionate Hospice Chaplain to provide spiritual and emotional support to patients and families facing end-of-life care. You’ll work within an interdisciplinary team to ensure comfort, dignity, and connection throughout the care journey.

The role emphasizes non-denominational, inclusive care, developing individualized spiritual plans, and bereavement support, with a full-time weekday schedule.

Qualifications

• Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
• A minimum of one ACPE-accredited Clinical Pastoral Education (CPE) unit is required or preferred

Responsabilités

• Provide spiritual and emotional support to patients, families, and caregivers in alignment with beliefs and values.
• Offer non-denominational, inclusive care that respects all faiths and backgrounds.
• Participate in patient care conferences with the interdisciplinary team.
• Assess spiritual needs and develop individualized care plans.
• Conduct visits, prayers, blessings, and rituals as requested.
• Provide bereavement support and counseling to families.
• Build relationships with local faith leaders and resources.
• Maintain timely documentation of assessments, interventions, and outcomes.
