Responsibilities

  • Collaborate with the patient’s attending physician and Hospice physician to determine clinical eligibility for hospice care.
  • Assess patient medical status, plan and implement immediate medical interventions on admission day.
  • Coordinate all aspects of patient care with the interdisciplinary team (IDG).
  • Develop initial and updated Plans of Care with the IDG and physicians.
  • Provide patient/family counseling and education.
  • Participate in QAPI programs and in-service trainings.
  • Arrange medications and DME for same-day delivery.
  • Complete admission documentation and first 24 hours of care.
  • Communicate admissions information to Case Manager and other disciplines.
  • Provide emotional/physical support to patients and families.

SCHEDULE: MONDAY - FRIDAY, 9am - 6pm

Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.

The Hospice Admission Nurse performs consultation and admission visits to assess patient/family needs and collaborates with the patient's attending physician and the Hospice physician to determine the patient's clinical eligibility for hospice care.

RESPONSIBILITIES:

  • Responsible for collaborating with the patient's attending physician and the Hospice physician to assist him/her in determining the patient's clinical eligibility for hospice care
  • Responsible for assessing patient's medical status, planning, teaching and implementing immediate medical interventions on day of admission, as prescribed by physician's orders
  • Actively participates in coordination of all aspects of patient's care, in accordance with current professional standards and practice
  • Participates in ongoing interdisciplinary comprehensive assessment, as needed
  • Develops the initial Plan of Care (POC) and updated POC, as needed, in collaboration with the interdisciplinary group (IDG), the patient's attending physician, the Hospice physician, and the patient/family
  • Contributes to patient and family counseling and education
  • Participates in Quality Assessment Performance Improvement (QAPI) program and Hospice sponsored in-service
  • Coordinates medications and durable medical equipment (DME) needs for same-day delivery
  • Responsible for completion of all elements of admission process and first 24-hours of care
  • Performs timely administrative functions relating to admissions, clinical records, Plan of Care, and mandatory abuse reporting requirements
  • Communicates admissions information, issues and concerns in patient care meetings to assure smooth transition and continuity of care with other IDG staff, including a detailed report to the assigned Case Manager and other assigned disciplines
  • Provides consultation and emotional/physical support to patients/families regarding Hospice services and disease processes
  • Performs routine nursing visits when requested

Requirements

  • Current license to practice as a registered professional nurse in Georgia
  • Current and continued CPR certification
  • At least two years of recent hospice experience
  • Excellent verbal and written communication and problem-solving skills
  • Responsible valid driver's license; reliable, insured, automobile for making home visits required
