Capstone Hospice, LLC in Lilburn, GA is seeking an experienced RN Case Manager to deliver compassionate, patient-centered end-of-life care within a defined territory. The role involves autonomous nursing practice supported by a leadership team focused on well-being and patient outcomes.
You will conduct patient assessments, develop care plans, manage pain and symptoms, educate families, and coordinate with IDG to ensure continuity of care.

As a Hospice Case Manager, you will play a vital role in providing compassionate, end-of-life care. Your ability to connect with patients and families while delivering expert clinical support will define their experience in their final journey. As an RN Case Manager your daily impact will involve:

You are provided autonomy to manage your schedule while being supported by a leadership team that prioritizes your well-being and the outcomes of the patient and family. It is important that the RN Case Manager live within a reasonable commute to manage a case load within the designated territory of the Lilburn/Stone Mountain or Lawrenceville area. This position has the expectations of working Monday - Friday, 8:00am - 5:00pm and attending required IDG meetings every other week in the office while managing a typical case load of 10-14 home and community-based hospice patients.
