Job summary

The Arc Eastern Connecticut is seeking an Individual Home Support Specialist to provide direct support in Norwich, CT. You will assist participants with daily living skills, personal care, recreation, health needs, and community integration.

You will also document progress, help with individualized support plans, and ensure health and safety. The role includes transporting participants to activities and preparing meals while modeling respectful advocacy.

Qualifications

• Prior experience working with people with developmental and physical disabilities preferred.
• Ability to mentor and assist people in life skills including recreation, independence, communication, and health.
• Valid driver’s license required.
• High School Diploma or GED required.
• Able to speak and write in English.

Responsibilities

• Deliver training, support, and assistance in personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
• Participate in the design, implementation and monitoring of the participant's individualized support plan.
• Accurately administer prescribed medications to participants as ordered by physicians.
• Accurately complete required documentation including program data, incident reports, daily log, progress notes, and health records.
• Ensure participants' health and safety including prevention of abuse, CPR, First Aid, Heimlich, and evacuations.
• Provide transportation to recreational and community activities.
• Prepare participants' meals following published menus.
• Mentor and assist people in all areas of life skills including recreation, independence, communication, and health.
