Job summary

All Heart Home Care in Murfreesboro, TN is seeking a compassionate Registered Nurse (RN) to join our home health team in a full-time role. You will perform in-home assessments, develop care plans, and provide skilled nursing services in patients' homes while collaborating with physicians and the healthcare team.

The position requires TN RN licensure and CPR certification; a valid driver's license and reliable transportation are also needed.

Qualifications

Must hold a Tennessee RN license.
CPR certification required.
Valid driver’s license with reliable transportation and auto insurance.
Strong clinical assessment, communication, and organizational skills.
Ability to work independently and manage a patient caseload.
Home health experience preferred but not required.

Responsibilities

Perform in-home patient assessments and develop individualized plans of care.
Provide skilled nursing services per physician orders and care plans.
Monitor patient conditions and communicate changes to physicians and care team.
Administer medications, treatments, and wound care as needed.
Educate patients and caregivers on disease management, medications, and home care practices.
Supervise and coordinate care by LPNs and other clinical staff.
Ensure accurate, timely documentation per policies and regulatory requirements.
Coordinate care with physicians, therapists, and other healthcare professionals.
Travel to patient homes within the assigned service area.
