Job summary

All Heart Home Care in Murfreesboro, TN is seeking a compassionate Registered Nurse (RN) to join our home health team in a full-time role. You will perform in-home assessments, develop care plans, and provide skilled nursing services in patients' homes while collaborating with physicians and the healthcare team.

The position requires TN RN licensure and CPR certification; a valid driver's license and reliable transportation are also needed.

Qualifications

• Must hold a Tennessee RN license.
• CPR certification required.
• Valid driver’s license with reliable transportation and auto insurance.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a patient caseload.
• Home health experience preferred but not required.

Responsibilities

• Perform in-home patient assessments and develop individualized plans of care.
• Provide skilled nursing services per physician orders and care plans.
• Monitor patient conditions and communicate changes to physicians and care team.
• Administer medications, treatments, and wound care as needed.
• Educate patients and caregivers on disease management, medications, and home care practices.
• Supervise and coordinate care by LPNs and other clinical staff.
• Ensure accurate, timely documentation per policies and regulatory requirements.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Travel to patient homes within the assigned service area.

All Heart Home Care is seeking a compassionate and experienced Registered Nurse (RN) to join our home health team in the Murfreesboro area. This full-time position provides the opportunity to deliver high-quality, patient-centered care in a home setting while working with a collaborative healthcare team.

Key Responsibilities:

• Perform in-home patient assessments and develop individualized plans of care
• Provide skilled nursing services in accordance with physician orders and established care plans
• Monitor patient conditions and communicate changes to physicians and the care team
• Administer medications, treatments, and wound care as needed
• Educate patients and caregivers on disease management, medications, and home care practices
• Supervise and coordinate care provided by Licensed Practical Nurses and other clinical staff as appropriate
• Ensure accurate and timely documentation in compliance with company policies and regulatory requirements
• Coordinate care with physicians, therapists, and other healthcare professionals
• Travel to patient homes within the assigned service area
