Job summary

AdvisaCare is seeking a Full-Time Home Health Registered Nurse to join our South East Grand Rapids team. You will provide skilled nursing care to patients in their homes, develop care plans, and coordinate with families and the clinical team to ensure optimal outcomes.

The ideal candidate will hold a Michigan RN license, have at least 1 year of home health experience, possess strong communication skills, reliable transportation, CPR certification, and the ability to pass screenings.

Qualifications

• Current and valid Registered Nurse license for Michigan
• Minimum of 1 year nursing experience, preferably in home health care
• Ability to work independently and manage a diverse patient caseload
• Strong communication and interpersonal skills
• Reliable transportation and valid driver’s license
• CPR certification
• Ability to pass drug screening and background check

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing treatments and procedures as required
• Maintain accurate and up-to-date patient documentation and records
• Coordinate care and communicate effectively with patients, families, and interdisciplinary teams
• Ensure compliance with safety, infection control, and regulatory standards
• Participate in patient education and advocate for patient needs
