AdvisaCare is seeking a Full-Time Home Health Registered Nurse to provide skilled nursing care in the South East Grand Rapids area, Hastings, MI. You will assess patients, create care plans, and collaborate with families and care teams to support patients in their homes.

Qualifications

• Current and valid Michigan RN license.
• Minimum 1 year of nursing experience, preferably in home health.
• Ability to work independently with a diverse caseload.
• Strong communication and interpersonal skills.
• CPR certification.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing treatments and procedures as required.
• Maintain accurate patient documentation and records.
• Coordinate care and communicate effectively with patients, families, and interdisciplinary teams.
• Ensure compliance with all safety, infection control, and regulatory standards.
• Participate in patient education and advocate for patient needs.
