Job summary

Infor in Michigan seeks an RN to provide comprehensive home health care using the full nursing process in patients' homes. You will assess, plan, implement and evaluate care, coordinating with physicians and families to meet physical, emotional, spiritual, and social needs.

You will document promptly, drive in varying weather, and participate in continuous quality improvement, while collaborating with a multidisciplinary health care team to deliver high-quality service.

Qualifications

• Successful completion of nursing program.
• Current licensure in the State of Michigan as a Registered Nurse (BLS from ARC or AHA).
• One (1) year of recent clinical experience.

Responsibilities

• Provides direct nursing care in the home setting using the nursing process.
• Coordinates patient/family care across disciplines with the physician.
• Completes all required documentation in a timely manner.
• Maintains a safe and risk-free environment for patients and families.
• Participates in quality improvement activities across the agency.
• Collaborates with the team to implement policies and procedures.
• Emphasizes orienting health care team members and building partnerships.
