Job summary
Mission Healthcare is seeking a registered nurse for full-time home health in Ventura, CA. You will perform initial patient assessments, develop and adjust care plans, and provide nursing care in patients' homes consistent with physician orders and best practices.

Qualifications
- Current and valid state RN license.
- Minimum of 1 year of recent acute care experience in an institutional setting.
- BLS with hands-on CPR certification.
- Valid driver's license and auto insurance.
- Associate's or Bachelor's degree in Nursing; 1 year home health experience preferred.
Responsibilities
- Completes initial assessment of patient and family to determine home care needs.
- Initiates and revises the plan of care as patient status changes.
- Regularly re-evaluates patients’ needs.
- Uses health data to determine nursing diagnoses.
- Develops care plans with goals based on diagnoses; involves patient and family.
