Job summary

Mission Healthcare is seeking a registered nurse for full-time home health in Ventura, CA. You will perform initial patient assessments, develop and adjust care plans, and provide nursing care in patients' homes consistent with physician orders and best practices.

Ideal candidates hold an active RN license with acute care and home health experience, BLS/CPR certification, and a valid driver's license. Competitive benefits and a stable, mission-driven team await.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Associate's or Bachelor's degree in Nursing; 1 year home health experience preferred.

Responsibilities

• Completes initial assessment of patient and family to determine home care needs.
• Initiates and revises the plan of care as patient status changes.
• Regularly re-evaluates patients’ needs.
• Uses health data to determine nursing diagnoses.
• Develops care plans with goals based on diagnoses; involves patient and family.

Pay range (depending on experience): $50 to $55 per hour

Schedule/Shift: Full time Light, Monday-Friday

Territory/Location: Ventura, CA

Responsibilities:

• Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Qualifications:

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands‑on CPR certification.
• Valid driver's license and auto insurance.
• Associate's or Bachelor's degree in Nursing, and 1 year of home health experience preferred.
