Job summary

CVHCare, a leader in home health clinical services, is seeking a Registered Nurse for weekend field work in California. This full-time role covers areas including Agoura Hills, Calabasas, Moorpark, Thousand Oaks, and more, with training and onboarding in Los Angeles.

The RN will manage patient care in the home setting, coordinating with physicians and the internal support team to ensure safe, personalized care plans and education for disease management.

Qualifications

• Minimum 1 year RN experience in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License
• Current CPR or BLS Card

Responsibilities

• The Home Health RN monitors a patient’s overall health and partners with a primary care physician to develop an individualized plan of care.
• Functions as a case manager and delivers quality healthcare in the patient’s home.
• Provides disease management education and services such as wound therapy and post-surgical care as needed.

Title:Weekend Home Health RN (Field)
Schedule: Full Time (24 visits per week w/benefits)
* Non-traditional schedules are available, i.e., Thursday-Monday
Locations:Area #45 covers the following cities:Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Villageand Area #46 covers the cites:Fillmore, Oxnard, Santa Paula, Somis, Camarillo
Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm and go to our Los Angeles office for onboarding

Position Summary:The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management.

We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.

Requirements:

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
• Must Possess an active CA Registered Nursing License
• AA, BSN, MSN
• Reliable Transportation
• Current CPR or BLS Card
