Job summary

AccentCare, Inc. is seeking a Registered Nurse Case Manager for our Home Health team in Minnesota. This full-time role covers Oakdale and surrounding areas, combining direct patient care with care coordination to support patients from admission through discharge.

You will lead and mentor staff, assess nursing needs, develop and monitor plans of care, and collaborate with physicians. A valid RN license, CPR certification, and at least one year of RN experience are required.

Qualifications

• Graduate from an approved school of professional nursing.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage patient's care plan.
• Complete initial patient assessment on admission with history.
• Select nursing diagnoses and coordinate interventions.
• Review medications for effectiveness, safety, and allergies.
• Establish goals aligned with diagnosis and communicate with physicians.
• Implement nursing interventions and document progress and discharge notes.
• Reassess patient needs and inform team of changes.
