Job summary

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes as developed by a Physical Therapist.
• Perform non-complex manual exercises, massage, heat/cold and modalities as prescribed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
