Job summary

Top Dog Recruiting seeks a dedicated Physical Therapist for a full-time role in Denver, delivering home health care to diverse patients. You will evaluate, plan, and document care in alignment with physician orders and agency policies.

Minimum nine months of PT experience required, with preference for home/community health background. Must hold a CAPTE-accredited degree and a current state license, CPR, and valid driver’s license.

Qualifications

• Nine months minimum experience as a physical therapist.
• Preference for community/home health experience.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations according to agency policy; selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of assessment results and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans, selecting intervention approaches, strategies and materials and establishing priorities.
• Provides organized patient treatment including advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team informed of status and plan.
• Provides treatment within patient needs and agency/payer parameters.
• Determines measurable goals in collaboration with team and patients.
• Meets agency productivity standards and uses work hours efficiently.

Education

Master’s degree in physical therapy CAPTE accredited program
