Job summary

ABL Health Care in Walnut Creek is seeking a full-time Physical Therapist (PT) to join our Home Health team. The PT will assess patients, develop care plans, and deliver skilled intermittent therapy in patients' homes, coordinating with our Clinical Director and field staff.

We support clinicians with back-office help, modern tools, and a benefits package including health, dental, vision, 401(k), and paid time off. Travel within a 15–25 mile radius is required for full-time staff.

Qualifications

• Current PT License to practice in California – Required.
• Experience in Home Health or Hospice care – 1 year required.
• Current CA Driver's License.
• Willingness to travel within a 15-25 mile radius.

Responsibilities

• Provide skilled intermittent home health care and educate patients and caregivers to promote maximal functional status.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor patient conditions, and report changes to the physician or nursing leadership.
• Initiate and revise the patient's plan of care (POC).
• Maintain accurate clinical records using company devices.
• Comply with all policies and procedures to ensure high standards of care.
