Job summary

AccentCare in Longview, TX is seeking a Physical Therapist for Home Health to evaluate and treat patients as part of an interdisciplinary team serving the Tyler, TX area. This is a full-time, on-site role with travel to patient locations.

The position requires CAPTE‑accredited PT education, a minimum of nine months of experience, Texas licensure, current CPR, and the ability to travel to all business locations.

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Currently licensed to practice as a physical therapist in Texas.
• Must possess and maintain valid CPR certification.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic materials to assess patient care.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals and priorities.
• Educates and counsels patients and families.
• Keeps physicians and team informed of patient status and treatment plan.
• Meets agency productivity standards and uses work hours efficiently.
