Preferred Care Home Health is hiring a Full-Time Physical Therapist (PT) for Naples, FL. The PT will evaluate patients in their homes, develop individualized treatment plans, and provide skilled therapy services per physician orders.

Responsibilities include progress monitoring, caregiver education, and thorough clinical documentation. Ideal candidates hold an accredited PT degree, an active license, and a valid driver’s license.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active, unrestricted Physical Therapist license in good standing.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and established care plans.
• Monitor patient progress and adjust interventions to support optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety techniques, and assistive equipment.
• Complete accurate and timely clinical documentation.
• Communicate changes in patient condition to physicians and the interdisciplinary care team.
• Provide clinical supervision to Physical Therapist Assistants (PTAs).
• Participate in case conferences, care planning, and ongoing clinical education.
