Job summary

Preferred Care Home Health in Naples, FL is hiring a Full-Time Physical Therapist (PT) to provide in-home rehabilitation services. The PT will perform evaluations, develop treatment plans, and educate patients and caregivers while documenting progress and coordinating with a care team.

The role requires an active PT license and a valid driver’s license. Home health experience is preferred but not required, with strong clinical judgement and communication essential for success.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active, unrestricted PT license in good standing.
• Home health experience preferred but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and established care plans.
• Monitor patient progress and adjust interventions to support optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive equipment.
• Complete accurate and timely clinical documentation.
• Communicate changes in patient condition to physicians and the interdisciplinary care team.
• Provide clinical supervision to Physical Therapist Assistants (PTAs).
• Participate in case conferences, care planning, and ongoing clinical education.
