Are you looking for a challenging and rewarding position as a Home Health and Hospice LPN? We are searching for compassionate, dynamic, energetic, organized, and efficient professionals with Home Health and Hospice experience to become part of our Home Health and Hospice Family servicing our clients in the Charlevoix and surrounding areas!

For this role, you need to be a licensed practical nurse who values home health and hospice care. You will be responsible for providing skilled nursing care to patients in their homes, as well as emotional support and comfort to patients and their families. Additionally, you will conduct supervisory visits to observe the performance of team members, participate in interdisciplinary team meetings, and contribute to developing, reviewing, and revising the patient/family plan of care.

• Initial and ongoing assessment of patients’ physical, functional, psychosocial, and environmental needs
• Implement the individualized plan of care and revisions as needed
• Ensure compliance with AdvisaCare Policies and Procedures and Medicare regulations
• Consult and educate patients and families on disease processes, self-care, end-of-life care, and ethical issues
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes, participate in IDG conferences
• Participate in performance improvement programs
• Maintain high customer satisfaction with clients, families, and caregivers
• Communicate effectively using current organizational processes and technology

Requirements:

• Current Michigan license in good standing
• Minimum of 1 year home health and hospice experience
• Empathetic approach to patients and caregivers
• Ability to participate in field visits and attend weekly office meetings
• Adaptability to patient needs and status
