Job summary

Hospice & HomeCare of Reno County is seeking a Home Health Aide (HHA) to provide direct patient care and supportive personal care per the patient’s plan of care, under licensed nursing supervision.

The role requires completion of an approved HHA program in Kansas, current HHA certification, CPR/BLS, and a valid Kansas driver’s license with auto insurance. Benefits include parental leave, tuition reimbursement, paid time off, and more in a compassionate hospice setting.

Qualifications

• Successful completion of an approved Home Health Aide (HHA) program in the state of Kansas.
• Current certification as Home Health Aide (HHA) in the state of Kansas. If CNA, HHA required within 3 months of hire.
• Current BLS/CPR card from approved provider within 30 days of hire or by end of orientation.
• Kansas Driver's License (if position requires driving) and Auto Insurance as per requirements.

Responsibilities

• Perform acts within scope of practice as defined by the applicable certifying board.
• Demonstrate knowledge and skills to provide care per unit/department competencies, policies, and procedures.
• Accurately evaluate patient needs and develop/implement/review/maintain a comprehensive care plan.
• Evaluate and provide care as directed to achieve desired patient outcomes from newborn to geriatric.
• Observe and report to hospice team patient/family physical, emotional and psychosocial status, disease process, symptoms and signs of death when appropriate.
• Complete timely and accurate documentation per Health System/affiliate policy.
• Act in accordance with the established mission, vision, and values.
• Abide by PHI privacy and security regulations.
• Maintain effective communication with patients, families, team members, and providers.
