Job summary

Addus HomeCare, Inc. is seeking a Registered Nurse to deliver professional nursing care to patients in home health settings under a FLEXIBLE SCHEDULE. Full-time, Monday–Friday, 8 am–5 pm, covering Sussex and Kent counties in Delaware.

As an RN with home health experience, you will perform wound care, infusion therapy, medication management, and patient teaching, plus supervisory visits for nonskilled clients and meticulous documentation to support high-quality care.

Qualifications

• Graduation from an accredited school of nursing and current RN licensure.
• Home Health experience required.
• Travel locally in your own vehicle with valid driver's license and insurance.

Responsibilities

• Perform wound care, infusion therapy, patient teaching, medication management.

• Maintain accurate and timely documentation.

As an Addus Registered Nurse, you will:

• Skilled Nursing Home Health Case Management.
• Perform Wound Care, Infusion Therapy, Patient Teaching, Medication Management.
• Performing Supervisory Nursing Visits for Nonskilled Clients.
• Maintain Accurate and Timely Documentation.
• Provide Education and Guidance to Clients and Caregivers.
• Support Quality Assurance and Risk Management Initiatives.
• Communicate Effectively with Staff, Caregivers, Families, and Referral Partners.
• Maintain a Collaborative Relationship with Clients, Families, Providers, and Care Team.
• Able to Ensure Compliance with Federal and State Regulations, Medicare and Medicaid Requirements, HIPAA standards, and Company Policy
• Performs other duties as assigned.

REQUIREMENTS

• Graduation from an accredited school of nursing and current RN licensure.
• Home Health experience required
• Familiarity with PointCare, Home Care Home Base is a plus.
• Experience with OASIS preferred.
• Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
