You will provide person-centered support to help individuals live with pride, dignity, and independence, making meaningful choices in daily life. Responsibilities include assisting with daily living activities, community involvement, and transportation, plus documenting care and following safety procedures.

Qualifications

• Must be at least 18 years old and meet background checks.
• High school diploma or equivalent preferred; not mandatory.
• Valid driver’s license with acceptable driving history.

Responsibilities

• Provide person-centered support based on individual needs and goals.
• Assist with daily living activities and personal care when needed.
• Support community involvement, recreation, appointments, shopping, and more.
• Encourage independence, informed choices, and skill development.
• Assist with meal prep, routines, and maintaining a safe home.
• Provide transportation as assigned and complete documentation on time.
• Follow policies, training, and health and safety procedures.
• Protect privacy, dignity, and rights of the people supported.
