Responsibilities

• Provide person-centered support based on each person’s goals and needs.
• Assist with daily living activities and personal care when needed.
• Support community involvement, appointments, shopping and activities.
• Encourage independence and informed choices.
• Assist with meal preparation and maintaining a safe home environment.
• Provide transportation as assigned.
• Complete required documentation accurately and on time.
• Follow PDI policies, training and safety procedures.
• Protect the rights and privacy of the people supported.
