Job summary

Choice Health at Home is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our Scottsdale home health branch. You’ll work with an Occupational Therapist to help patients regain independence and perform daily activities.

Responsibilities include carrying out plans, documenting progress, educating patients and families, and adhering to infection control and agency policies. A valid COTA license and CPR are required.

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) license.
• CPR certification.
• Valid driver’s license and auto insurance.
• Home health or skilled nursing facility experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Carry out treatment plans developed by the Occupational Therapist.
• Help patients build functional skills and work toward their therapy goals.
• Document care accurately and communicate progress to the Occupational Therapist and care team.
• Educate and encourage patients and families throughout treatment.
• Follow infection control practices and agency policies to support safe, consistent care.
