Qualifications

• Illinois CNA certification and active status on the Illinois Health Care Worker Registry.
• Previous experience in a skilled nursing facility or long-term care setting preferred but not required.

Responsibilities

• Assist residents with activities of daily living (bathing, dressing, grooming, toileting, eating).
• Provide personal care while maintaining dignity, privacy, and independence.
• Assist with transfers, repositioning, ambulation, and mobility.
• Monitor and report changes to nursing staff; document vital signs and intake/output.
• Respond to call lights promptly and assist with meals and hydration.
• Maintain a clean, safe, and comfortable environment; follow care plans and infection control policies.
• Support fall prevention and resident safety; communicate with the care team.
