Job summary

Sequoia Transitional Care in Porterville, CA, seeks a Staff Physical Therapist to join our therapy department on a full-time or part-time basis. The role emphasizes evaluating and treating residents, collaborating with the care team, and delivering high-quality, patient-centered rehabilitation.

The ideal candidate holds a CA PT license, CPR certification, and has excellent communication and documentation skills.

Qualifications

• Valid CA Physical Therapy License is required.
• Current CPR Certification is required.
• Experience in geriatrics or rehabilitation medicine is preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to relate effectively with patients, families, and interdisciplinary team.

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Deliver treatment in accordance with physician orders and PT best practices.
• Supervise Physical Therapy Assistants in accordance with state regulations.
• Maintain detailed, accurate, and timely documentation of all patient care.
• Participate in interdisciplinary meetings including Care Conferences and Utilization Reviews.
• Provide caregiver education and training for post-discharge care.
• Recommend and assist with ordering of durable medical equipment (DME).
• Ensure all department equipment is maintained in good working condition.
• Conduct in-service education for facility staff.

Position Summary
