You will evaluate patients, develop individualized treatment plans, supervise assistants, participate in case management, and ensure care complies with state regulations and Synergy Care guidelines. This role emphasizes compassionate, evidence-based outpatient therapy and collaborative practice.

Qualifications

• Graduate of an accredited Physical Therapy program (or graduating within 6 months).
• NBCOT certification required.
• Current or eligible state PT license required.
