Job summary

Mt. Washington Pediatric Hospital is seeking a Family Self-Sufficiency (FSS) Coordinator to provide housing-based case management and workforce development support for Public Housing, HCV, and Homeownership participants under HUD FSS guidelines.

The role reports to the FSS Supervisor, requires a 4‑year degree, and offers an exempt, full-time position in Louisville. Primary duties include ITSP development, escrow support, and coordinating services to promote financial empowerment.

Qualifications

• Bachelor’s degree from an accredited institution in social work, human services, public administration, or a related field.
• Minimum of 2 to 3 years of experience in PHA self‑sufficiency programs and case management.
• Must successfully pass a criminal background screening.

Responsibilities

• Conduct participant assessments to identify needs and goals for participants.
• Maintain accurate and confidential information.
• Monitor participant progress towards goals through regularly scheduled contact/appointments.
• Develop, implement, and monitor Individual Training and Services Plans (ITSPs) in compliance with HUD FSS requirements.
• Coordinate referrals to employment, education, and financial empowerment partners.
