Job summary

4th Second, a Solano County nonprofit focused on enhanced care management and youth programs, seeks a Community Health Outreach Worker to connect unhoused individuals with medical, mental health, and housing services. Qualifications

• High School Diploma or GED required; CHW Certification may be substituted.
• Six months of full-time community outreach or related support experience required.
• Driver’s license and access to reliable transportation required.

Responsibilities

• Conduct face-to-face fieldwork in streets, encampments, and shelters to build trust.
• Perform street-level screenings and connect clients to services.
• Coordinate care and referrals with health and social service systems.
• Document clinical data and maintain HIPAA-compliant records.
• Advocate for clients and support housing stability and employment goals.
