Job summary

Amae Health on the Upper East Side and Brooklyn is hiring a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join an interdisciplinary team delivering personalized care to individuals with serious mental illness. You will evaluate, treat, and coordinate care across psychiatry, therapy, primary care, and social services.

Requirements include an advanced nursing degree, PMHNP-BC, NY NP license, and DEA; minimum two years working with SMI/outpatient populations; willingness to obtain

Qualifications

• Master's or Doctoral degree in Nursing required.
• PMHNP-BC board certification required.
• Active, unrestricted New York State NP license and prescriptive authority.
• DEA registration or ability to obtain.
• Clozapine REMS certification and Medicaid enrollment, if needed.
• Minimum 2 years working with SMI/outpatient.
• Familiarity with NYC area health and social services systems.
• Holistic health perspective and patient rights awareness.
• Ability to work with diverse populations.
• Strong interpersonal and communication skills; adaptable in a dynamic environment.
• Comfort with technology to improve care delivery.

Responsibilities

• Provide behavioral healthcare management to Amae Health patients; monitor and manage care through various treatment modalities as patients progress through recovery phases.
• Conduct psychiatric evaluations and ongoing medication management for patients with serious mental illness
• Develop individualized treatment plans addressing psychosocial and medical needs that influence recovery
• Provide continuous, regular consultations as patients navigate recovery, both in person and occasionally virtually
• Liaise with patient care teams and collaborate with behavioral health providers for integrated care
• Address medical and behavioral health needs to mitigate social determinants of health effects
• Educate patients and families about mental health conditions, treatment options, and recovery planning
