Job summary

Amae Health in Los Altos, CA seeks a Psychiatric Mental Health Nurse Practitioner to join an interdisciplinary team delivering personalized, high-quality care for adults with serious mental illness. Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• PMHNP-BC board certification.
• Active California NP license with prescriptive authority.
• DEA registration or ability to obtain.
• 2+ years working with SMI/outpatient.
• Familiarity with LA Metro area medical/mental health systems.
• Holistic view of health and patient rights.
• Strong interpersonal and communication skills and adaptability.
• Comfort with technology and digital care delivery.

Responsibilities

• Provide behavioral healthcare management across recovery phases for Amae Health patients.
• Conduct psychiatric evaluations and ongoing medication management.
• Develop individualized treatment plans addressing psychosocial and medical needs.
• Provide regular consultations in person and, when needed, virtually.
• Collaborate with patient care teams to ensure integrated care.
• Address medical and behavioral health needs to mitigate social determinants of health.
• Educate patients and families about mental health conditions, treatments, and recovery planning.
