Job summary

Destination Independence LLC seeks compassionate caregiver support for clients living at home in San Antonio. You will follow individualized care plans, monitor health, and report observations to the Case Manager to ensure safe, steady progress.

Duties include assisting with daily activities, meal preparation, transportation to appointments, social engagement, and emotional support to help clients stay independent and mentally alert.

Responsibilities

• Maintain records of patient care, condition, progress, or problems and report observations to the Case Manager.
• Interact, support, and converse with clients to keep them mentally healthy and alert.
• Provide clients and families with emotional support and instruction for living independently or adapting to disability.
• Perform duties as requested, such as obtaining household supplies or running errands.
• Accompany clients to doctors' offices or on trips outside the home, providing transportation and companionship.
• Services will be provided to all clients when needed.
• Prepare meals according to diet or medical needs (diabetic, weight loss, etc.).
• Ensure proper attire according to medical recommendations and climate.
• Report any maladaptive behaviors.
• Report any suspected abuse, neglect, and exploitation to the Case Manager immediately.
• Assist access to medical, educational, and other services, including transportation to Day Habilitation/Work location.
• Approach individuals with a positive attitude to build self-esteem.
• Recreational and community activities with prior approval.

We provide services that assist an individual in gaining access to needed medical, educational, and/or other services, including Transportation to Day Habilitation / Work location / Teaching ADLs according to a developed plan to reach a higher level of Independence.

Approach individuals with a positive attitude to build a higher level of self–esteem.
