Job summary

Forward Family Care, LLC in Wisconsin seeks a dedicated Caregiver to support young people and adults with disabilities, promoting independence while ensuring safety and dignity in daily life.

You will assist with ADLs, meals, transportation, and meaningful activities, following care plans and HIPAA requirements. Flexibility for evenings/weekends is valued; growth opportunities exist within a supportive community.

Qualifications

  • Experience in group homes, assisted living facilities, nursing homes, or home health environments preferred.
  • HIPAA knowledge to ensure client confidentiality and privacy.
  • CPR and First Aid certification required; other certifications are a plus.
  • Ability to perform patient observation and assist with transfers safely.

Responsibilities

  • Assist clients with activities of daily living (ADLs) such as personal care, grooming, and hygiene.
  • Support clients during outings, appointments, and transportation using safe transfer methods.
  • Prepare nutritious meals aligned with care plans and dietary needs.
  • Maintain a clean, safe environment adhering to health standards.
  • Implement care plans including medication reminders and dementia/autism support as required.
  • Engage clients in meaningful activities to foster social, cognitive, and emotional well-being.

  • Assist clients with activities of daily living (ADLs) such as personal care, grooming, and hygiene to promote independence and comfort
  • Support clients during outings, appointments, and transportation using appropriate manual handling and patient transfer methods like Hoyer lifts or other assistive devices
  • Prepare nutritious meals tailored to individual dietary needs, including cooking and meal planning aligned with care plans
  • Maintain a clean, safe environment by performing light cleaning duties and ensuring adherence to health and safety standards
  • Implement individualized care plans that include medication reminders, dementia or Alzheimer’s care, memory care, or autism support as required
  • Engage clients in meaningful activities that foster social interaction, cognitive development, and emotional well-being while respecting resident’s rights and preferences
  • Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
  • Knowledge of HIPAA regulations to ensure client confidentiality and privacy is maintained at all times
  • Certification in CPR and First Aid is required; additional certifications such as medication administration or hospice care are a plus
  • Demonstrated ability to perform patient observation, monitor vital signs, and assist with patient transfer methods safely
  • Experience working with individuals with developmental disabilities, neurodevelopmental disorders, dementia, or intellectual disabilities support preferred
  • Strong communication skills for effective client interaction and team collaboration
  • Ability to lift heavy objects safely using manual handling techniques such as Hoyer lifts or patient transfer devices
