Qualifications

• Master's degree in a human services field with supervised child-placing experience or equivalent.
• Alternative combos include 350 hours field placement or practicum in social services.
• Experience requirement ranges from 2 to 3 years depending on degree path.

Responsibilities

• Provide ongoing monitoring, support, and case management for foster/adopt families and clients.
• Document visits, service plan goals, and actions within required timelines (two business days).
• Coordinate and attend ARD or school-related meetings to meet educational needs.
• Schedule, facilitate, and document service plan meetings and progress.
• Respond to referrals and prepare families/children for placement; conduct follow-ups post-placement.
• Assist with behavioral interventions and collaborate with supervisors on plan development.
• Attend court hearings and communicate with legal representatives and treatment teams.
• Maintain confidentiality and compliance with DFPS standards and Pathways policies.
