The Village Network in Youngstown, OH is seeking a Clinical Case Manager for Foster Care. You will provide clinical support aligned with our mission, developing service plans and coordinating with therapists to monitor patient progress.

Responsibilities include case management, court and school meetings, and supporting therapy services, with opportunities for professional development and licensure reimbursement.

Formación

• Bachelor's Degree in Social Work, Counseling or related field required; LSW or LPC licensure recommended.
• Valid Ohio Driver’s License with insurable driving record.
• Willingness to travel and visit clients' homes.
• Excellent verbal and written communication; strong teamwork and time management.

Responsabilidades

• Facilitates case management per goals and objectives identified by therapists.
• Develops and writes service plans for direct placements.
• Reviews materials and reports from outside organizations and monitors treatment milieu.
• Participates in court hearings and school IEP meetings; leads team meetings.
• Supports individual, family, and group therapy; crisis counseling; staff training and transports.
