Universal Health Services (UHS) in partnership with Greystone Foster Care is seeking a Case Manager Supervisor in Brentwood, TN. This hybrid role blends remote work with in-office requirements and occasional court appearances.

Qualifications

• Master's degree in Social Work or human services is required.
• Five to eight years of related experience and/or training.
• Valid state Driver's License is required.
• Automobile insurance with verification of medical liability.

Responsibilities

• Set standards and expectations for case management paperwork quality, accuracy and timeliness.
• Ensure compliance with contractual regulations and agency policies for foster care/in-home services.
• Provide clinical consultation and review admissions/referrals as needed.
• Communicate with the Director regarding case assignments at intake.
• Perform random chart audits for quality documentation and audit readiness.
• Schedule meetings among involved parties and with medical appointments for quality care.
• Facilitate child and family team meetings for program planning.
• Maintain weekly, monthly, and quarterly records; review monthly progress reports and IPPs.
• Visit each assigned child per policy or as needed.
• Report non-compliance or abuse to proper authorities; attend court hearings as appropriate.
• Provide training for orientation to new Case Manager employees; support development plans.
• Review and approve outside training for Case Manager staff.
• Monitor client movement and prepare discharge planning reports.
• Approve time-off requests and provide direct individual and family therapy when needed.
