Job summary

FamilyForward is seeking a Foster Care Case Manager to support vulnerable children and families through comprehensive therapeutic and educational services. Qualifications

• Bachelor’s degree required.
• Experience working in foster care and/or with at-risk youth and families.
• Experience providing case management services (preferred).
• Team player with collaborative approach.
• Commitment to achieving best outcomes for children and families.
• Ability to manage multiple demands, make independent decisions, and manage time/resources effectively.

Responsibilities

• Make decisions related to client services for medical, education, social, and emotional supports; ensure access to identified resources.
• Maintain rotating on-call availability for emergencies.
• Form permanency goals and plans for assigned clients.
• Prepare records and documentation for clients, including assessments and treatment plans.
• Lead/participate in monthly Family Support Team meetings with parents, child, and caregiver as required.
• Attend court hearings when requested by supervisor.
• Transport clients to appointments, court, school, and events as needed.
• Attend all monthly team meetings and coordinate Family Support Team activities.
• Identify and coordinate links to community resources for clients.
