Job summary

Arrow Child & Family Ministries is seeking a Case Manager to coordinate treatment and services for foster/adopt children and their families in Texas. The role includes developing plans, monitoring licensing compliance, and maintaining records.

You will work closely with foster families, schools, and social services to ensure holistic care, crisis support, and timely documentation while upholding agency standards.

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• Minimum 1 year of experience with children and families.
• Master’s degree preferred.

Responsibilities

• Facilitate placement of children in foster/adoptive homes and complete intake paperwork within policy timeframes.
• Develop assessment, treatment planning, service delivery, discharge and aftercare per service package.
• Assist and monitor families for licensing/regulatory compliance and credentialing.
• Implement Pre-Admission Assessment and Individual Treatment Plans with foster parents and clients.
• Provide consultation for foster/adoptive/biological families and caregivers as indicated.
• Maintain accurate case notes, billing docs, and treatment plans per standards.
• Attend trainings and meetings; serve as education liaison with referral agency and foster parents.
• Coordinate case planning conferences, school meetings, and other cross-system activities.
• Maintain regular contact with family and child; monitor well-being via calls, visits, and emails.
• Contribute to a therapeutic milieu; provide crisis intervention 24/7 as needed.
• Coordinate care with medical consenter and STAR Health where applicable.
• Complete discharge summaries and after-care services per service package.
• Maintain records in Arrow’s electronic client information system; use mobile data access for documentation.
