Job summary

Gulf Coast JFCS is seeking a Case Manager to provide direct case management services for children, birth families, foster families, and primary caregivers to ensure access to appropriate services and continuity of care.

You will assess, plan, coordinate, advocate, and maintain relationships with all parties, including attendance at court dates and required meetings, while upholding confidentiality and meeting policy standards.

Qualifications

• Bachelor's degree in a related field is required.
• An Associate's degree with 60+ college credits plus two years of full-time social work or human services experience is acceptable.

Responsibilities

• Identify strengths, limitations, risk factors, and needs to refer and coordinate services.
• Maintain face-to-face contact with children and others as required by state laws and policy.
• Notify parties of status changes and ensure attendance at court dates and meetings.
• Provide information and accompany children to appointments as necessary.
• Transportation and supervision of children and participants, including those with medical/behavioral health needs.
• Provide mentoring, training, and crisis intervention to foster parents and caregivers.
• Maintain confidentiality per policy and law.

Education
