Lutheran Social Services Of The Nat in Fairfax, VA is seeking a Contractor social worker to provide foster care case management for children and youth placed in care within the Fairfax County/Falls Church service area.

You will coordinate services with DSS, foster families, and other providers, ensure documentation, and maintain confidentiality while meeting contract requirements. The role requires driving within the service area and is a day shift contractor position.

Formación

• Bachelor's or Master's degree in Social Work or related field.
• At least one year of experience in case management or social services.
• Knowledge of trauma-informed and culturally responsive practices.
• Spanish/English bilingual preferred.

Responsabilidades

• Provide foster care case management to children and youth in coordination with DSS.
• Conduct face-to-face and/or virtual visits and maintain ongoing case contact.
• Assess client needs and develop, monitor, and update service and permanency plans.
• Coordinate placement, medical, behavioral health, educational, legal, and community services.
• Participate in placement planning, case conferences, and team meetings.
• Complete timely case records, reports, and required documentation.
