Job summary

Children's Hope Alliance is seeking a Foster Care Case Manager in North Carolina. You will support children and foster families, coordinating services and guiding teams through admission, treatment, and discharge planning.

You will provide coaching to foster parents, conduct home visits, and advocate for families. Maintaining records, ensuring licensing compliance, and responding to urgent needs are also part of the role.

Qualifications

• Bachelor's degree in psychology, social work, or related field is required.
• At least 2 years of experience working with children or families.
• Qualified Professional (QP) designation is required.
• Ability to respond to urgent situations and participate in on-call rotation.

Responsibilities

• Guide children and foster families through the foster care journey.
• Coordinate services, treatment planning, and resources.
• Facilitate Child and Family Team meetings and contribute to admission, treatment, and discharge planning.
• Provide ongoing coaching and hands-on support to foster parents.
• Conduct regular in-home visits to assess needs and ensure a safe environment.
• Respond to urgent needs and participate in on-call rotation.
• Maintain accurate documentation and case records.
• Monitor licensing compliance and review medical records.

What You'll Do

Champion Care for Youth & Families

• Serve as a trusted partner to children, foster families, and care teams-guiding them throughout the foster care journey
• Coordinate services, treatment planning, and resources to support each child's unique needs
• Facilitate Child and Family Team Meetings and actively contribute to admission, treatment, and discharge planning

Support & Empower Foster Families

• Provide ongoing coaching, training, and hands-on support to foster parents
• Conduct regular in-home visits to assess strengths, address needs, and ensure a safe, nurturing environment
• Help families navigate behavioral challenges using proven models like the Teaching Family Model

Be a Consistent Advocate

• Maintain strong communication with biological families, social workers, and community partners
• Represent Children's Hope Alliance in court hearings, treatment reviews, and external meetings
• Respond to urgent needs and provide crisis/on-call support when it matters most

Ensure Quality & Compliance

• Keep accurate, timely documentation and case records using clinical management systems
• Monitor foster home compliance with licensing standards and conduct ongoing reviews
• Review medical administration records to ensure the safety and well-being of children
