Responsibilities

Develops and monitors child-specific treatment plans.
Facilitates quarterly treatment team meetings and coordinates visits.
Coordinates visits with biological family to promote connections and permanency.
Maintains case records and ensures timely documentation.
Collaborates with DSS, providers, and families to coordinate services.
