Job summary

Massachusetts Mentor, a part of the Sevita family, is seeking a Foster Care Case Manager to coordinate services, manage case plans, and oversee care quality for youth and families in our residential and treatment foster care programs.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of work with individuals with disabilities or at‑risk youth preferred.
• Equivalent combination of education and experience accepted.
• Valid driver’s license, vehicle, insurance, and reliable transportation.
• Willing to travel and work on-call (nights/weekends).
• Reliable, compassionate attitude and commitment to quality.

Responsibilities

Foster Care Case Manager

Salary - $62,000 (+ Overtime pay)

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
• Assist in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.

Qualifications

• Bachelor's degree in human services or related field.
• One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc., preferred.
• An equivalent combination of education and experience.
• Current driver's license, car registration, auto insurance, reliable transportation, and clear driving record is required.
• Willing to travel is required.
• Flexible work schedule is required.
• Willing to work on-call (after hours and weekend hours) schedule is required.
• A reliable, responsible attitude and a compassionate approach.
