Job Description for Care Coordinator

What will you do?

The primary purpose of this position is to oversee the provision of Family Foster Care & Therapeutic Foster Care Level I & II services. Responsibilities include clinical supervision and training of providers, monitoring service delivery for outcomes and compliance, using the System of Care approach for consumers up to age 21. Duties may combine social work, case coordination, and licensing responsibilities.

Responsibilities include:

  1. Ensuring Client Rights are adhered to for all consumers.
  2. Implementing all policies, procedures, regulations, and standards of the agency.
  3. Representing CTS Inc. professionally in the community.
  4. Following agency, state, and federal rules, including quality improvement, medical records, and finance.
  5. Monitoring quality service delivery aligned with best practices.
  6. Providing case management, assessments, and care level appropriateness.
  7. Monitoring clients' mental and physical health needs and reporting concerns.
  8. Meeting Medicaid record rules and deadlines.
  9. Coordinating services with treatment team members.
  10. Conducting child family teams and developing intervention plans.
  11. Ensuring service plan requirements are met and updated annually.
  12. Completing SAR requests and maintaining authorization continuity.
  13. Providing 24-hour crisis support, participating in staffing, and training therapeutic families.

Pay Rate: $45,000 - $55,000

Qualifications:

  • Completion of a 4-year bachelor’s degree in human services and at least 2 years’ full-time experience with MH/DD/SA populations.
  • No substantiated abuse or neglect findings; no criminal convictions.
  • Qualified Mental Professional status.
  • Valid North Carolina Driver’s License; ability to drive in the Charlotte area.
