Job summary

Presbyterian Children's Homes and Services (PCHAS) seeks a Foster Care Case Manager to ensure safety and services for children in foster care. You will coordinate family support, individualized treatment plans, and provider activities across home, school, and community settings, documenting progress to support permanency for children and families.

The role requires a bachelor's in a human services field (master's preferred) and 2+ years of case management experience, with flexible work locations

Qualifications

• Must possess a bachelor's degree in social work, psychology, counseling, education, criminal justice, or a related human services field.

Responsibilities

• Facilitate Family Support Team meetings and development of individualized care plans.
• Coordinate services across home, school, and community settings.
• Document support activities and services in required databases (FACES and CSWIS).
• Maintain monthly face-to-face contact with each assigned child and family.
• Attend court hearings and coordinate Family Support Team meetings within required timeframes.
• Participate in FCCM consortium on-call schedule as EOC.

Education

Bachelor's degree in social work/psychology/counseling/education/criminal justice/related human services field

Master's degree preferred
