Job summary

ICES: Data, Discovery, Better Health in Region 6 seeks a Family Support Specialist to work with active DFPS cases in home-based and field assignments. You will monitor foster/adoptive families, manage documentation, and coordinate services with the care team.

The role emphasizes a strengths-based approach, strong writing, and detailed organization to ensure safety, compliance, and positive client outcomes. Training and collaboration with CPS are integral to the position.

Qualifications

• Bachelor’s degree in a human services field from an accredited college or university.
• Experience working with individuals and families that have active DFPS cases preferred.
• Ability to be clear headed and decisive.
• Impeccable writing skills; detailed and organized; positive client interaction.

Responsibilities

• Monitor Foster and Adoptive Home for safety and compliance.
• Record all information for home and child files weekly in the agency’s database.
• Complete initial placement of children and youth.
• Monitor children's progress in foster/adoptive homes.
• Coordinate STAR Health service coordination and set up required assessments.
• Manage a caseload of 25–30 cases.
• Schedule and conduct Treatment/Service Plan Meetings with CPS and team members.
• Complete required monthly, quarterly, and annual documentation.
• Connect family to social work for community resources and supports.
• Provide aftercare case management and crisis intervention as needed.
